Patient Enrollment Specialist

• Conduct outbound calls to eligible patients to introduce the @HOME program and encourage patients to participate in the innovative home-based care model
• Work closely with teams to continuously identify new and creative ways to connect with patients to engage in the program

• Track record of success in health-care-related engagement, sales, and/or marketing
• Ability to independently foster relationships with patients and their caregivers
• A passion, patience, and ability to explain complex topics to seniors and their caregivers
• Ability to educate patients about numerous care services accessible through their health plan or their PCP
• A commitment to performing meaningful work that dramatically improves the lives of older adults
• An understanding of the Medicare and Medicare Advantage programs
• A demonstrated ability to operate independently in a fast-paced environment
• Excellent communication, follow-up, teamwork, and problem-solving skills
• Computer skills: Ability to quickly navigate and use multiple computer programs to include, but not limited to: Gmail, MS Word or Google Docs, Excel or Google Sheets, etc.
• Bilingual (English and Spanish) a plus
• US Work Authorization is required
