Job Description:

The Patient Engagement Specialist will be part of the Population Health Team and responsible for conducting patient outreach via telephone and in-person home visits to foster engagement in Oak Street Health’s care model. The role includes supporting patients in accessing care, documenting interactions while maintaining confidentiality, and fostering relationships with patients to improve healthcare outcomes.

Job Responsibility:

  • Conduct patient outreach (telephonic and in-person home visits) to foster engagement in the care model
  • Engage with patients who have not visited the clinic or been engaged in the care model
  • Spend time in the community connecting with patients in locations such as homes, hospitals, long-term care facilities, and skilled nursing facilities
  • Respond to escalations and urgent needs while prioritizing work, triaging situations, and multitasking
  • Document interactions with patients in electronic medical records while maintaining HIPAA standards
  • Communicate complex topics with all identified parties involved in patients' care, including family members, caregivers, and medical providers
  • Partner with patients and their support persons to identify goals and address barriers
  • Foster relationships with patients and their caregivers
  • Partner with Community Health Workers, Social Workers, and clinicians to support complex patient needs
  • Educate patients about Oak Street Health’s wrap-around services

Requirements:

  • A track record of success in health-care-related engagement, sales, and/or marketing
  • Strong verbal and written communication skills
  • Strong technology skills and understanding of Excel as well as other EHR/EMR platforms
  • Dedication to serving the community and building meaningful relationships
  • A demonstrated ability to operate independently in a fast-paced environment
  • An understanding of the Medicare and Medicare Advantage programs
  • Access to reliable transportation and the ability to travel through assigned territories to connect with patients
  • US Work Authorization is required

Nice to have:

Fluency in Spanish (written and oral)
