Job Description:

The Patient Engagement Specialist plays an integral role in the Population Health Team by fostering engagement in care models, conducting outreach, and supporting patients to access healthcare services. The role involves connecting with patients in community settings and addressing a range of priorities in collaboration with healthcare professionals and social workers.

Job Responsibility:

  • Conduct patient outreach (telephonic and in-person home visits) to foster engagement in care model
  • Engage with patients who have not visited the clinic or care model to support their access to care
  • Plan to spend half the time connecting with patients in patient-centered locations such as homes, hospitals, long-term care facilities, and skilled nursing facilities
  • Respond to escalations and urgent needs, prioritize work, triage situations, and multitask
  • Document interactions with patients in electronic medical records while maintaining HIPAA standards
  • Communicate complex topics between identified parties involved in patient care
  • Partner with patients and their support persons to identify goals and address barriers
  • Foster relationships with patients and caregivers
  • Partner with Community Health Workers, Social Workers, and clinicians to support complex patient needs
  • Educate patients about Oak Street’s wrap-around services

Requirements:

  • A track record of success in health-care-related engagement, sales, and/or marketing
  • Strong verbal and written communication skills
  • Strong technology skills and understanding of Excel as well as other EHR/EMR platforms
  • Dedication to serving the community and building meaningful relationships
  • A demonstrated ability to operate independently in a fast-paced environment
  • An understanding of the Medicare and Medicare Advantage programs
  • Access to reliable transportation and the ability to travel through assigned territories to connect with patients
  • US Work Authorization is required

Nice to have:

  • Fluency in Spanish (written and oral)
