Patient Engagement Specialist

• Conduct patient outreach (telephonic and in-person home visits) to foster engagement in care model
• Engage with patients who have not visited the clinic or been engaged in care model
• Assist patients in finding appropriate primary care settings
• Respond to escalations and urgent needs with ability to prioritize work
• Document interactions with patients in electronic medical records
• Communicate complex topics between all parties involved in patients' care
• Partner with patients to identify goals and address barriers
• Foster relationships with patients and their caregivers
• Partner with Community Health Workers, Social Workers, and clinicians
• Educate patients about wrap-around services

• Track record of success in health-care-related engagement, sales, and/or marketing
• Strong verbal and written communication skills
• Strong technology skills and understanding of Excel as well as other EHR/EMR platforms
• Dedication to serving the community and building meaningful relationships
• Demonstrated ability to operate independently in a fast-paced environment
• Understanding of the Medicare and Medicare Advantage programs
• Access to reliable transportation and ability to travel through assigned territories
• US Work Authorization
