Title: Patient Engagement Specialist (Fluent in Polish). Location: Hybrid, Harwood Heights area. Role Description: The Patient Engagement Specialist I will serve as an essential member of the Patient Engagement and Population Health Team. This role necessitates conducting patient outreach, encompassing telephonic communication and in-person home visits, to cultivate engagement within our care model. The PES I is responsible for engaging patients who have not yet visited the clinic or participated in our care model, providing support to facilitate their access to care at Oak Street. Furthermore, the PES I may assist patients in identifying a more suitable setting for their primary care requirements.
