Job Description:

Our client is looking for a dedicated Patient Care Coordinator to join a dynamic team in Trevose, Pennsylvania. In this contract position, you will play a pivotal role in ensuring patients receive exceptional home health care services. This opportunity offers flexibility and the chance to make a meaningful impact on patients' lives.

Job Responsibility:

  • Coordinate patient care by collaborating with field staff, healthcare providers, and external organizations
  • Conduct training sessions and orientations for new caregivers to ensure readiness and compliance
  • Monitor and enforce caregiver clock-in and clock-out procedures to maintain accurate records
  • Request necessary authorizations and prepare care plans to support patient needs
  • Visit patients in their homes as required for effective care management and assessment
  • Build strong relationships with service coordinators to enhance service delivery
  • Develop and maintain patient schedules, assigning caregivers promptly and efficiently
  • Work to improve patient satisfaction and caregiver retention through effective communication and support
  • Address patient concerns and provide assistance to ensure a high level of care
  • Ensure all administrative tasks related to patient medical records are completed accurately

Requirements:

  • Minimum of one year of experience in home care preferred
  • Ability to maintain a detail-oriented appearance and demeanor in all interactions
  • Strong work ethic and commitment to delivering quality care
  • Excellent oral and written communication skills, along with active listening abilities
  • Capability to work independently, maintain confidentiality, and meet deadlines
  • Effective interpersonal skills combined with sound judgment and decision-making abilities

Nice to have:

  • Bilingual proficiency in Spanish is preferred but not required
  • Familiarity with patient scheduling and medical records systems is a plus
