We are looking for a Patient Care Coordinator to support daily patient access and administrative operations for a healthcare team in New Hyde Park, New York. This Contract position focuses on creating a smooth experience for patients by coordinating appointments, maintaining accurate records, and responding to routine care-related questions. The ideal candidate is organized, service-driven, and comfortable handling scheduling tasks while supporting front-end patient coordination in a fast-paced clinical setting.
Job Responsibility:
Coordinate patient visit calendars by arranging, adjusting, and confirming appointments based on provider availability and patient needs
Welcome and assist patients during scheduling and check-in activities, helping address routine questions with professionalism and empathy
Maintain and update patient medical documentation to ensure records remain complete, accurate, and easy to retrieve
Review appointment details for accuracy, including patient information, visit timing, and service requirements before scheduled encounters
Communicate with patients regarding upcoming visits, rescheduling needs, and general preparation instructions when appropriate
Support day-to-day administrative workflow by organizing patient information and helping keep care coordination processes efficient
Verify and record relevant patient details during interactions to promote accurate scheduling and streamlined service delivery
Requirements:
Experience coordinating patient appointments in a medical, hospital, or outpatient setting
Working knowledge of patient medical records and the importance of maintaining accurate documentation
Ability to manage scheduling activities with strong attention to detail and time sensitivity
Comfortable assisting patients in person or by phone with a courteous and patient-focused approach
Strong organizational and communication skills suited to a fast-moving healthcare environment
Ability to check and confirm patient information accurately during intake or scheduling processes
Proficiency with standard office systems and healthcare administrative tools used for patient coordination
