Job Responsibility

Provide Direct Patient Safety: Specifically monitor patients who may be confused, impulsive, at risk for injury, or require close observation
Support Under RN Direction: Work closely with a Registered Nurse to create a safe and therapeutic environment
Deliver Compassionate Care: Offer companionship and emotional support, contributing to the spiritual, physiological, social, and psychological well-being of patients
Documentation & Communication: Accurately document observations and communicate effectively with clinical staff
Respond to Needs: Set priorities for response and support clinical staff in emergency situations

Job Responsibility

Ensures a secure and safe environment by maintaining a continued awareness of safety hazards and observation of the patient
Never leaves assigned patient unattended, ensures that patient is continuously monitored before leaving room
Follows established policies and procedures for precautions, i.e., suicide, seizure, falls, elopement, restraining, provides restraint alternatives when appropriate
Follows established policies and procedures for emergency plans and codes
Accompanies assigned patient at all times, i.e., walks in the hallway to various ancillary departments, bathroom, shower, etc
Provides general patient care duties that are specific to the patient(s) served as directed by the registered nurse
Provides am or pm care to include making occupied or unoccupied bed, bed bath, oral and denture care, shave back rub, and skin care
Assists patient with ordered activity, i.e., ambulation, getting up to chair and back to bed, positioning, etc
Assists with toileting needs of the patient including care of the incontinent patient and proper diaper usage
demonstrates awareness of patients with high risk of skin breakdown

Assures alarms are activated with appropriate settings and are sufficiently audible.
Maintains continuous visual observation of multiple patients at one time on a monitor
assures a co-worker is monitoring assigned patients during breaks or other temporary situations when unable to maintain continuous observation (e.g., when engaged in two-way video call)
Redirects patients promptly to maintain patient safety.
