Job Description

St Francis Catholic College is seeking an experienced and student-focused Pathways Coordinator to lead the delivery of careers and pathways education at our Cobblebank Campus. This leadership role supports students in making informed decisions regarding subject selection, further education, training and employment pathways, ensuring every learner is prepared for life beyond school.

Job Responsibility

  • Lead the delivery of careers and pathways education at our Cobblebank Campus
  • Support students in making informed decisions regarding subject selection, further education, training and employment pathways
  • Provide individual pathways counselling including Year 12 exit interviews
  • Coordinate key initiatives such as work experience and careers events
  • Build strong partnerships with tertiary providers and industry

  • Assist in developing early MDT processes, documentation templates, SOPs, and workflow standards
  • Set up MDT schedules, digital platforms, meeting templates, and information-sharing pathways
  • Ensure MDT structures and processes align with NCCP tumour-site and regulatory standards
  • Support preparation for “go live” and contribute to ongoing service development
  • Schedule and organise MDT meetings, circulate case lists, and ensure all relevant information is available in a timely manner
  • Liaise closely with radiology, pathology, medical oncology, radiation oncology, surgery, and nursing teams to ensure complete case preparation
  • Record MDT discussions and ensure all minutes of the MDT discussions and outcomes are reflected within the patient records accurately and within a timely manner
  • Disseminate decisions and MDT patient outcomes to the relevant teams within a timely manner
  • Facilitate virtual, hybrid, or in-person MDT formats. Prioritise in-person MDT meeting formats
  • Monitor MDT member attendance and ensure this is communicated to the chair on a quarterly basis
