Job Description:

Signify Health, part of CVS Health, is hiring part-time clinicians to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. These evaluations include reviewing medical history, conducting physical evaluations, and performing diagnostic tests as requested by the health plan.

Job Responsibility:

  • Conduct pre-scheduled in-home visits with health plan members
  • provide at least 12 hours of availability weekly
  • execute a strong visit completion rate
  • conduct video visits as needed
  • maintain required supplies and equipment
  • complete diagnostic and preventive service products
  • re-engage members with their healthcare during visits

Requirements:

  • Active state licenses in coverage area
  • board certification
  • experience with Signify Health In-Home Health Evaluations or similar format (preferred)
  • ability to travel locally and within licensed states (up to 25%)
  • ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
  • required training and credentialing for all health plan clients and lines of business
