The CSR1–Outreach Representative provides face-to-face enrollment services, outreach, and education to beneficiaries about their Managed Care Options. This role supports community engagement, assists individuals with health plan enrollment/disenrollment, and maintains strong relationships with county staff and partner organizations.
Job Responsibility:
Conduct in-person outreach, presentations, and enrollment services for beneficiaries (group and individual settings)
Explain Managed Care Options and assist beneficiaries in understanding and using their health plans
Support enrollment/disenrollment processes and provide status updates to beneficiaries
Assist with onboarding guidance for new Enrollment Service Representatives
Build and maintain positive relationships with county staff, community groups, and HCO clients
Provide ongoing program training to county staff
Complete data entry and transmit enrollment/disenrollment data per HCO procedures
Assist beneficiaries with medical exemption and waiver inquiries
Document all daily activities accurately in the CRM database
Review knowledge base content and recommend improvements
Meet all Field Operations performance and production standards
Report quality issues and procedural concerns promptly
Support continuous improvement by making workflow/process recommendations
Attend community events and health fairs as needed
Work in any assigned office location as directed by management
Perform other related duties as assigned
Develop working relationships with customers and track engagement and progress
Assist customers through various stages of benefit and service applications
Conduct workshops and training for participants
Coordinate services to support program goals (e.g., medical, childcare, transportation)
Follow up with customers to resolve needs, questions, and concerns
Maintain accurate, timely case notes and documentation
Share outreach findings and potential service barriers with project staff
Collaborate with team members to resolve participant issues
Requirements:
High School Diploma, GED, or equivalent
Bilingual in English/Spanish (Fluency, reading and writing)
Knowledge of local community resources
Computer literacy
proficiency with MS Office (Word, Excel, PowerPoint)
Ability to learn new software quickly
Type at least 20+ WPM
Valid California driver's license and proof of insurance
Ability to travel reliably throughout Santa Clara County
Strong communication, interpersonal, and organizational skills
Ability to work in a fast-paced, deadline-driven environment
Able to manage multiple complex tasks simultaneously
Capable of working both independently and as part of a team
Nice to have:
Six months of experience in human services or customer-facing roles
Experience with presentations, teaching, outreach, marketing, or public speaking
Experience working with low-income or diverse populations
Ability to present information in a culturally and linguistically appropriate manner
