Job Description:

The Outreach Executive (OE) is responsible for developing relationships with members of the community to engage Medicare seniors with Oak Street Health. Key responsibilities include engaging Medicare seniors to convert them into patients, and working closely with Outreach Managers, Directors, and Community Relationship Managers to drive patient growth.

Job Responsibility:

Developing relationships with members of the community
engaging Medicare seniors with the goal of onboarding them as patients of Oak Street Health
collaborating with Outreach Managers, Outreach Directors, and Community Relationship Managers to drive growth locally

Requirements:

Mandarin or Cantonese fluency
ability to engage Medicare seniors to become patients of Oak Street Health
collaboration with Outreach Managers, Outreach Directors, and Community Relationship Managers to drive new patient growth
40 anticipated weekly work hours
