Job Description:

The Outreach Executive is tasked with developing relationships within the community to engage Medicare seniors and encourage them to become patients of Oak Street Health. This role involves collaboration with Outreach Managers, Directors, and Community Relationship Managers to drive local patient growth.

Job Responsibility:

Develop relationships with members of the community
engage Medicare seniors to become patients of Oak Street Health
collaborate with Outreach Managers, Outreach Directors, and Community Relationship Managers
drive new patient growth locally

Requirements:

Engage Medicare seniors with the goal of having those individuals become patients of Oak Street Health
work closely with Outreach Managers, Outreach Directors, and Community Relationship Managers
drive new patient growth locally in assigned territory
