Job Description

The Outreach Caseworker position is a full time, outreach position providing services to Adults residing in the community, experiencing chronic behavioral health issues. (Diagnoses may include, substance use disorder, schizophrenia. dual diagnosis, schizoaffective disorder, physical issues, etc.) The Caseworker works with a multi- disciplinary team that includes Master level clinicians, nursing staff, peer specialists and others involved in client care.

Job Responsibility

  • Provide community outreach and be responsible for the welfare of a large case load of clients
  • Advocate for clients ensuring quality of care
  • Recommend appropriate resources for clients ensuring access to needed services
  • Make assessments
  • address client needs, make recommendations for treatment
  • Serve as primary liaison with family and guardians
  • Work closely with team of providers
  • May provide transportation
  • administer medication
  • manage client finances
  • Provide documentation
  • update and maintain client health records at Braintree office site using an electronic health record system

Requirements

  • HS Diploma
  • BA degree in Psychology or related field preferred
  • One or more years of experience in the Mental Health field
  • Working knowledge of community based treatment, funding sources, and treatment regulations is strongly preferred
  • Current valid driver's license in good standing, driving experience and an excellent driving record
  • CPR/First Aid certification
  • MAP certification

  • BA degree in Psychology or related field
