Requirements

Licensed as a registered Nurse in the Commonwealth of Massachusetts, or State of New Hampshire as appropriate
Baccalaureate degree in nursing strongly preferred. If associate degree in nursing, must complete BSN degree within 4 years
Minimum of 1-year recent experience as an RN
Ambulatory and/or oncology experience preferred
Professional nursing certification preferred (OCN, BMTCN, CPHON, AOCNP, CRNI, RN-BC)
AHA BLS required (AHA ACLS accepted as minimum requirement)
Demonstrated ability to work as an effective member of an interdisciplinary team

Active RN license

Job Responsibility

Patient Relationship & Assessment: Build connection and trust with patients at the first point of contact. Assess medical, psychosocial, and logistical needs

Care Coordination: Collaborate closely with the care team to schedule timely consults, tests, and supportive appointments while navigating payer and authorization processes
Clinical Triage: Review patient records and apply clinical knowledge to confirm eligibility, appropriate specialty placement, and readiness for first appointment
Patient & Family Education: Provide education about diagnoses, treatment pathways, and support resources to help patients and caregivers make informed decisions
Documentation & Communication: Maintain detailed, accurate documentation in Epic. Ensure efficient information flow among team members for continuity and safety of care
Program Development: Participate in quality improvement initiatives that enhance the patient and caregiver experience, streamline navigation workflows, and elevate MD Anderson's intake standards
Professional Development: Stay current with navigation best practices and oncology trends through continuing education and internal training opportunities

Escort patients during their DFCI main campus appointments and sit in on clinical visit as needed
Remind patients of appointments and follow-up to maximize attendance
Assist patients with scheduling appointments, coordinating referrals, and managing schedules
Provide navigation services to patients pursing enrollment by identifying and address barriers to participate
Identify system and/or organizational barriers to care and recommend strategies to improve/remove them
