Job Description:

The Oncology community health nurse is a professional nurse knowledgeable in Oncology nursing who provides specialized care to patients in the home. Flexible per diem VNA position with requirement of 1 weekend shift (SAT and SUN) every 4 weeks and 1 holiday per year.

  • Delivers long-term survivorship care to gynecologic oncology patients who have completed active treatment
  • Evaluates existing gynecologic oncology patients in collaboration with gynecologic surgical and medical oncology physicians
  • Participates in shared decision-making with patients and caregivers
  • Focuses on screening and prevention through personalized and comprehensive risk assessments
  • Designs individualized imaging and surveillance plans in collaboration with the multidisciplinary team
  • Incorporates evidence-based risk reduction strategies and health promotion into care planning
