Job Responsibility
Undertaking specialized assessments for older individuals experiencing complex mental health issues and cognitive decline
Creating tailored, risk-managed care plans that promote autonomy and protect vulnerable adults
Working closely with Community Mental Health Teams (CMHTs), psychiatrists, and memory clinics to ensure holistic care
Executing mental capacity assessments and best interest decisions where appropriate
Requirements
Social Work England (SWE) Registration is essential
Solid experience working within an Adult Mental Health setting or a dedicated older persons specialist service
In-depth understanding of the Care Act 2014, Mental Health Act 1983 (amended 2007), and the Mental Capacity Act 2005
Exceptional crisis-management skills and the ability to build trust with vulnerable service users
