This is an exciting opportunity for an Occupational Therapist to join our team working with Portsmouth residents and their careers in their own homes.
Job Responsibility:
Assess needs, identify outcomes and implement agreed action plans using a strength-based model
Adapt methods of carrying out daily living activities to support individuals to facilitate choice and control
Enable participation in the community, promoting independent living and Wellbeing
Use assistive technologies, specialised equipment and adaptations to enable individuals to restore occupational performance
Provide support and guidance to clients and their families in their adjustment to disability and long-term conditions
Monitor and adapt action plans to ensure effectiveness and achievement
Complete and document risk assessments to ensure safe practice
Provide education and advice to clients, carers and professionals as required
Work independently and be responsible for managing your own caseload
Manage case records within an environment offering a high level of support and supervision
Be fully conversant with statutory legislation and guidance and implement this in day-to-day practice
Requirements:
Diploma of the College of Occupational Therapists or BSc Occupational Therapy or any Occupational Therapy qualification recognised by the world federation of Occupational Therapist
Current registration with HCPC
Knowledge of Adult Social Care eligibility criteria
Clinical reasoning and problem-solving skills
Ability to work independently using initiative whilst managing a varied caseload under pressure
Strong communication and interpersonal skills
Competence in IT systems and electronic record keeping
Ability to travel across Portsmouth and access clients homes
Flexible and responsive to changing needs and demands
