Health Care Professions Council registration, licence to practice
Evidence of on-going CPD appropriate to this role
Evidence of achievement of appropriate competencies relevant to this role
Knowledge and experience of a wide range of approaches to the management of complex conditions demonstrated by clinical reasoning
Evidence updated with professional practice and current best evidence and new research
Excellent communication skills demonstrating an ability to communicate to a variety of audiences

Job Responsibility

Provide specialist assessment of patients functional abilities in their own home or other community setting, in order to establish treatment plans and person centred goal orientated plans, alongside an MDT including Physiotherapists, Specialist Nurses, Speech and Language therapists and support staff
Deliver specialist treatment plans, including provision of specialist equipment, neuro splinting, cognitive assessment and rehabilitation, upper limb rehabilitation, ADL related goals, and specialist postural management
Engage in development and training within the team, through supervision with Band 7 OT, in-service training and other CityCare wide training opportunities
To be involved in service development and transformation of services to enhance service delivery for patients and their carers
