The Clinical Beds team provides holistic assessments to patients who require a period of rehabilitation and assessment prior going home after being discharged from hospital. We work closely as a multidisciplinary team alongside our partner services such as Adult social care and continuing health care to aid patients and families make long term decisions about their health and social care needs. This is an excellent opportunity to broaden your skill set in a holistic manner working with physiotherapists, occupational therapists and mental health nurses along with supporting junior colleagues such as student nurses, assistant practitioners and rehabilitation support workers.
Job Responsibility:
To work as an autonomous practitioner and undertake comprehensive assessments to identify the appropriate service for the clients referred
To carry out profession specific, and holistic, assessments and evaluation as required
To assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs either in a community bed
To be responsible for continuing the ongoing development of high standards of clinical practice with evidence based focus
Able to fulfil physical demands of role, including manual handling applications
To participate in rehabilitation treatment programmes with the MDT
To carry out training of other members of staff
Support senior colleagues with the facilitation of student placements
To contribute to a high quality service by carrying out assessment, goal setting, implementing treatment programmes and evaluation of adult clients who may have complex and/ or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists
To work within Community Beds Team withing Health Reablement service . This may extend or change to other community services in-line with organisational development
To support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health services, voluntary services and public sector agencies
To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training
To be responsible for planning a safe discharge from the service
To act as Key Worker as required
Requirements:

BSc/Diploma in Occupational Therapy
HCPC Registration
Evidence of ongoing professional Development
This post may require shift working between 8am-4pm, 12-8pm 5 days over 7 including weekend and bank holidays
Full driving licence, use of vehicle for work with business insurance
Relevant under graduate/placement experience working with adults including Health Care of the Elderly and Older peoples mental health / Integrated Medicine, Orthopaedics, Neurology and Musculoskeletal
Understanding of multi-disciplinary and/or multi-agency working in a community setting
Competence in individual caseload and time management
Ability to work under pressure, manage caseload. Customer service skills. Enthusiastic and motivated. Non-judgemental attitude, motivated. Flexible, adaptable, innovative, self-reliant and with a positive attitude. Ability to relate well to all groups of people and to create and maintain effective working relationships within the team and externally.
