Job Responsibility:

  • Evaluate referred residents to determine their functional level and potential for rehabilitation
  • Provide direct care and develop individualized treatment plans to help residents regain independence in activities of daily living (ADLs)
  • Establish treatment goals based on each resident’s abilities, limitations, and physician recommendations
  • Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills
  • Educate residents and their families on home programs and adaptive techniques
  • Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries
  • Collaborate with the interdisciplinary team to ensure a resident-centered approach to care
  • Adhere to all applicable federal and state regulations, including HIPAA and Residents' Rights
  • Perform additional duties as assigned by the Director of Rehabilitation.

Requirements:

  • Graduate of an accredited Occupational Therapy program
  • Valid Occupational Therapy license and registration for the state of practice
  • Experience in long-term care or geriatrics is preferred but not required
  • Strong communication and organizational skills
  • Compassionate, team-oriented, and committed to resident-centered care.
