Occupational Therapist

• Evaluate, diagnose, and treat adult and geriatric residents with physical, cognitive, and functional limitations in a SNF/LTC environment
• Develop and implement individualized, goal-driven treatment plans in collaboration with nursing, PT, SLP, and medical staff
• Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards
• Educate residents, families, and staff on adaptive techniques, equipment use, and energy conservation strategies
• Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes

• Current, unrestricted Occupational Therapist license in the state of practice (or eligibility prior to start)
• NBCOT certification preferred
• Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred
• Excellent communication, documentation, and collaboration skills
• Resident-centered, outcomes-focused approach
