Kanda is seeking a compassionate and experienced Occupational Therapist (OT) to join our Veterans’, Aged Care and Community Nursing Services team. This full-time role involves establishing and delivering Transitional Care Program (TCP) plans that meet clinical best practice and safety standards. You’ll work with the Clinical Manager and multidisciplinary team to provide holistic, person-centred care, promote independence, and support clients transitioning from hospital to home.
Job Responsibility:
Clinical Assessment & Care Planning: Conduct comprehensive assessments and develop tailored TCP and care plans in alignment with clinical best practice and individual goals
Rehabilitation & Functional Support: Provide direct client care including functional assessments, rehabilitation, and environmental safety evaluations focused on falls prevention and mobility improvement
Equipment Prescription & Management: Prescribe, document, and request DVA equipment within required timeframes. Manage stock, hire, and delivery of equipment, ensuring correct installation and staff/client education
Collaboration & Communication: Liaise effectively with hospital discharge planners, medical practitioners, funding bodies, and allied health providers to support seamless care coordination
Documentation & Compliance: Maintain accurate client records and ensure all documentation meets professional, regulatory, and organisational standards
Quality Improvement: Support annual audits and continuous improvement initiatives, actively contributing to the enhancement of clinical services and client outcomes
Requirements:
Bachelor of Applied Science (Occupational Therapy) and current AHPRA registration (no conditions)
Minimum 2 years’ experience in a clinical or community-based occupational therapy role
Proven experience in functional and environmental assessments, rehabilitation, and equipment prescription
Excellent communication and relationship-building skills with clients, families, and colleagues
Current Driver’s Licence and reliable vehicle are essential
Hold or willing to obtain: NDIS Workers Screening Clearance
