Job Description:

Join our team as a Band 6 Occupational Therapist and play a key role in improving the lives of elderly patients. In this role, you will perform assessments, create care plans, deliver interventions, and collaborate with multidisciplinary teams.

Job Responsibility:

Perform comprehensive assessments for older adults
Create and implement personalised care and rehabilitation plans
Deliver evidence-based interventions to optimise function and independence
Collaborate with multidisciplinary teams for holistic patient care
Maintain accurate and timely patient records

Requirements:

A current and valid HCPC registration
Experience working within care of the elderly / older adult settings
Strong communication and interpersonal skills
The ability to work independently and as part of a team
A commitment to high-quality patient care
