Job Responsibility:

Perform individualized evaluation with client or family to determine goals
Implement customized intervention to improve ability to perform daily activities and reach goals
Conduct outcomes evaluation to ensure goals are met or make changes to intervention plan
Adapt environment or tasks to fit the person with a holistic perspective

Requirements:

Current, unrestricted license in state of practice
