About the Role The Occupational Therapist provides comprehensive evaluations and therapeutic interventions for participants enrolled in Blue Ridge Independence at Home (PACE). This role partners closely with the interdisciplinary team (IDT) to develop individualized Plans of Care that support functional independence, safety, and overall well‑being. The Occupational Therapist delivers care in both center-based and home settings, ensuring services align with participant goals while supporting clinical excellence, regulatory compliance, and coordinated care delivery.
Job Responsibility:
Conduct initial and ongoing occupational therapy assessments for participants
Develop, implement, and adjust individualized treatment plans across acute, restorative, and maintenance care levels
Collaborate with the interdisciplinary team to coordinate comprehensive care plans
Communicate participant progress and changes to ensure continuity of care across settings
Provide education to participants, families, and caregivers on safety, functional mobility, and daily living skills
Deliver interventions to improve strength, endurance, cognition, and ability to perform activities of daily living (ADLs)
Recommend and fit adaptive equipment, orthotics, and assistive devices
Assess and coordinate home modifications to promote safety and independence
Train caregivers and staff to support participants’ functional abilities
Maintain accurate, timely clinical documentation in accordance with regulatory standards
Monitor participant progress and adjust care plans as needed
Participate in Quality Improvement initiatives and interdisciplinary meetings
Ensure compliance with safety, infection control, and organizational policies
Serve as a resource and clinical partner to interdisciplinary team members
Support training and competency development of staff as needed
Contribute to a collaborative, mission‑driven team environment focused on participant-centered care
Requirements:
Graduate of an accredited Occupational Therapy program
Current, unrestricted Occupational Therapist (OT) license in Virginia
Minimum of one (1) year of experience working with geriatric or frail populations
CPR certification
Valid driver’s license with reliable transportation
Strong communication, documentation, and collaboration skills
Ability to work both independently and within an interdisciplinary care model
Nice to have:
Experience in PACE, home health, geriatrics, or community-based care
Previous experience working within an interdisciplinary team model
Familiarity with adaptive equipment and home safety modifications
