We are seeking a dedicated Occupational Therapist to join this progressive team and provide person‑centered services in a senior living setting.
Job Responsibility
Evaluate, diagnose, and treat adult and geriatric residents with physical, cognitive, and functional limitations in a SNF/LTC environment
Develop and implement individualized, goal-driven treatment plans in collaboration with nursing, PT, SLP, and medical staff
Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards
Educate residents, families, and staff on adaptive techniques, equipment use, and energy conservation strategies
Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes
Requirements
Current, unrestricted Occupational Therapist license in the state of practice (or eligibility prior to start)
NBCOT certification preferred
strong new grads with relevant LTC/acute rehab experience may be considered based on client need
Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred
Excellent communication, documentation, and collaboration skills
