Job Responsibility:
Play a pivotal role in improving clients' independence, wellbeing, and quality of life
Perform assessments within clients' homes to understand their needs
Deliver clinical interventions, including recommending assistive technology and home adjustments
Provide expert advice, assessment, and prescription of assistive technology and home modifications for CHSP eligible clients
Assist with coordinating a case load, including planning, service delivery, and evaluation
Ensure accurate and up-to-date clinical documentation
Identify and complete referrals to other services as needed
Work in a hybrid role, involving in-home visits, office work, and remote work from home
Primarily based in the Metro North area, with consideration for other metro regions
Requirements:
Passionate about delivering compassionate, community-based aged care that promotes dignity and independence
Skilled in collaborating with clients, caregivers, and healthcare professionals to achieve positive client outcomes
Fully certified with AHPRA registration, NDIS Workers Screening, WA Driver’s License, and current First Aid & CPR
ICT skills (including use of databases and Microsoft Office suite applications)
Experience working with people with disabilities and/or experience with Assistive Technologies
Nice to have:
Experience within the Aged Care sector is preferred
