Requirements

Degree in Nursing, PT, OT, SLP or other health related field
Possession of a current license to practice in the residing state
One (1) to two (2) years of clinical home health experience required
Working knowledge of OASIS and ICD-10 coding
Possess and maintains current CPR certification
Detail oriented and able to work with minimal supervision
Strong communication skills and interpersonal skills
Valid driver’s license and proof of insurance is required
MUST HAVE A MULITSTATE [sic] license
Must have Home Health, and OASIS expereince [sic]

Job Responsibility

Prospectively reviews all OASIS assessments to ensure appropriateness, completeness, and compliance with federal and state regulations and organization policy
Ensures appropriate ICD-10 coding and sequencing as it relates to the patient’s medical condition, including any co-morbidities
Works with managers to address trends that affect the agency’s outcome and process measures noted during OASIS review
Makes OASIS inter-rater reliability visits with clinical staff
Participates in Quality Improvement and Corporate Compliance activities as assigned
Assists with other chart audit activities as assigned

Job Responsibility

Maintain accurate and complete financial records of the home in line with Company Policies and Procedures, using both computer and manual systems as required
Prepare and issue regular reports within prescribed timescales
To process receipts of monies against resident’s accounts and maintain accurate records
Prepare and submit relevant resident’s financial information to the Finance Department to ensure that invoices are produced accurately and promptly
Ensure continuity of payroll
Maintain staff attendance records and ensure rotas accurately reflect hours worked
Maintain complete files for both residents and staff in line with Advinia’s policies and procedures
Provide administration and secretarial support to the Home Manager e.g., typing, filing, dealing within correspondence etc. in line with prescribed timescales
Operate and maintain office equipment such as the fax, photocopier and computers calling in external assistance where required
To answer the telephone, respond to queries from residents and visitors and to re-direct enquiries in a friendly and efficient manner

Job Responsibility

Work with the Home Manager, Head of Dementia & Lead Happieness Ambassador to engage with and fully implement Advinia’s Happieness & Wellbeing programme and embed the whole home approach to activities
Support the Community Engagement Framework alongside the wider team to build community links and promote community events such as Intergenerational sessions, Coffee Mornings, Open Days and Cultural Celebrations
Develop, manage and coordinate a regular, varied calendar of group activities, in collaboration with the Residents, that are person-centred and meaningful to help maintain hobbies and interests and meet holistic needs
Plan individualised 1:1 Sessions for Residents as appropriate and engage with ongoing life story work to help inform the activity programme and person-centred care planning
Manage the activity budget in conjunction with the Home Manager to ensure the provision of resources, outings and entertainment and undertake fundraising opportunities to supplement as appropriate
Maximise the use of resources that are available to support with provision of meaningful engagement opportunities. Ensure risk assessments are completed for all activities and outings and reviewed as appropriate
Act as a Dementia Ambassador at your Location
Support all Colleagues to become Dementia Friends
Offer Signposting to family members to Admiral Nurse services, Local carer groups, tide, Alzheimer’s Society etc
Maintain stock of Dementia Publications
