The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to serve as a Health Facilities Surveillance Nurse in the Division of Assisted Living.
Job Responsibility:
Conduct surveys of facilities, including state licensure and complaint surveys
Prepare written survey documentation
Under general supervision, surveys facilities to determine compliance with state licensure requirements and Department directives, program policies and procedures
Travels to conduct routine health and safety surveys to determine compliance with state and federal requirements
Interviews facility residents, staff, administrators, health care providers and other individuals
Observes patient care provided at facilities
Reviews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance
Performs monitoring functions at noncompliant facilities
Completes necessary surveyor training
Provides technical assistance to facilities in emergencies
Participates in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed
Requirements:
Graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience
OR a bachelor’s degree in nursing and two (2) years of professional nursing experience
OR a master’s degree in nursing
Licensure as a Registered Nurse in the State of Illinois
Appropriate, valid driver’s license
Ability to travel in performance of duties with overnight stays as appropriate
Ability to work evenings and weekends
Must reside in St. Claire, Clinton, Washington, Monroe, Randolph, Perry, Jackson, Union, Alexander or Pulaski County
Ability to meet all agency vaccine/health-related polices and guidance
Nice to have:
One (1) year experience in assisted living care setting
Three (3) years professional nursing experience
Two (2) years’ experience participating in the survey process for completion of all survey functions including but not limited to annual recertification, licensure, follow-ups, complaints, and incident investigation surveys
One (1) year experience preparing complex written & oral reports
One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel PowerPoint, Smartsheet)
One (1) year experience exercising judgement, discretion and maintain confidentiality during the conduct of the survey process
Two (2) years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders
