Job Description

The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to serve as a Health Facilities Surveillance Nurse in the Division of Field Operations. This individual will conduct surveys of facilities, including state licensure, federal certification and complaint surveys. The HFSN will be responsible for preparing written survey documentation. Under general supervision, surveys facilities to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures. Travels to conduct routine health and safety surveys to determine compliance with state and federal requirements. Interviews facility residents, staff, administrators, health care providers and other individuals. Observes patient care provided at facilities. Reviews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance.

Job Responsibility

  • Review medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance
  • Perform monitoring functions at noncompliant facilities
  • Complete necessary surveyor training
  • Provide technical assistance to facilities in emergencies
  • Participate in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed

Requirements

  • Graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience
  • OR a bachelor’s degree in nursing and two (2) years of professional nursing experience
  • OR a master’s degree in nursing
  • Licensure as a Registered Nurse in the State of Illinois
  • Appropriate, valid driver’s license
  • Ability to travel in performance of duties with overnight stays as appropriate
  • Ability to work evenings and weekends
  • Must reside in Cook, DuPage, Grundy, Kane, Lake, McHenry, Kendall or Will County
  • Ability to meet all agency vaccine/health-related polices and guidance

Nice to have

  • One (1) year experience in long-term care setting
  • Three (3) years professional nursing experience
  • Two (2) years experience participating in the survey process for completion of all survey functions including but not limited to annual recertification, licensure, follow-ups, complaints and incident investigation surveys
  • One (1) year experience preparing complex written & oral reports
  • One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel PowerPoint, Smartsheet)
  • One (1) year experience exercising judgement, discretion and maintain confidentiality during the conduct of the survey process
  • Two (2)years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders

  • Processes Federal statements of Deficiency and Plans of Correction for inclusion on the Federal and Department websites
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above
