Job Description:

The Nurse Practitioner role at Oak Street Health involves providing effective and equitable value-based primary care to adults on Medicare. The position focuses on building meaningful relationships with patients through longitudinal care, collaborating with a care team, and participating in population health initiatives.

Job Responsibility:

Provision of exceptional primary care
Conduct office visits for routine and acute issues
Administer Annual wellness visits and health risk assessments
Care coordination with other providers, specialists, testing facilities, and agencies
Population health leadership
Assisting the care team with phone triage and outreach
Educating patients on their health conditions, care plans, and treatments
Participating in Oak Street Health promotional activities
Conducting home visits as needed
Other duties as assigned

Requirements:

Master of Science in Nursing
Doctor of Nursing Practice
NP Certificate Program graduate
National certification in Family Nurse Practitioner
National certification in Adult-Gerontology Primary Care Nurse Practitioner
National certification in Adult Nurse Practitioner
National certification in Gerontological Nurse Practitioner
Active state Nurse Practitioner license
Active DEA license
US Work Authorization

Nice to have:

Experience in primary care: internal medicine, geriatrics, or family medicine
Understanding of HCC documentation
Understanding of ICD-10 Coding
Understanding of Health Risk Assessments (HRAs)
Passion for teamwork
Interest in collaborating cross-functionally
Bilingual proficiency in applicable areas
