Job Description:

Oak Street Health Home Visit NPs deliver high-touch, high-quality primary care to homebound patients, supported by a team including a scribe/medical assistant and social worker. NPs conduct medical house calls, monitor clinical status, and collaborate with care teams to ensure compliance and promote the Oak Street Health care model.

Job Responsibility:

  • Conduct medical house calls for homebound patients according to defined pathways
  • optimize and monitor clinical status, identify and address gaps in care, reconciling medications and addressing adherence challenges
  • collaborate with home health, specialists, social workers, hospitals, SNFs, and specialists
  • ensure clinical excellence, compliance with regulatory guidelines, and promotion of the Oak Street Health care model
  • other duties, as assigned

Requirements:

  • Genuine passion for primary care and reaching under-resourced communities
  • strong clinical skills focused on older adults with complex disease states
  • experience in home-based care (as RN or APN)
  • excellent communication, collaboration, and problem-solving skills
  • desire to be a part of a fast-paced, innovative team
  • have an active, non-probationary state medical license, including US work authorization, if applicable
  • 1+ years APN experience, preferably with a Medicare population
  • access to reliable transportation
  • flexibility to travel around the local market
  • bilingual (English and Spanish) preferred
  • working knowledge of Word, Excel and EMRs
  • US work authorization
  • someone who embodies being 'Oaky'
