Job Description:

The Nurse Practitioner will provide value-based primary care to adults on Medicare in an outpatient setting, focusing on building meaningful patient relationships, promoting care coordination, and championing population health.

Job Responsibility:

  • Provision of exceptional primary care
  • Conduct office visits for routine and acute issues
  • Administer Annual Wellness Visits and health risk assessments
  • Care coordination with providers, specialists, testing facilities, and agencies
  • Population health leadership
  • Assist the care team with phone triage and outreach
  • Educate patients on health conditions, care plans, and treatments
  • Participate in Oak Street Health promotional activities
  • Conduct home visits as needed

Requirements:

  • Master of Science in Nursing
  • Doctor of Nursing Practice
  • NP Certificate Program graduate
  • National certification in at least one specialty
  • Active, non-probationary state Nurse Practitioner license
  • Active DEA license
  • US Work Authorization

Nice to have:

  • Experience in primary care: internal medicine, geriatrics, or family medicine
  • Understanding of HCC documentation, ICD-10 coding, and HRAs
  • Passion for teamwork and cross-functional collaboration
  • Interest in innovative, population health-focused models
  • Bilingual proficiency in applicable areas
