Job Description:

The purpose of the Nurse Practitioner role is to provide effective and equitable value-based primary care to adults on Medicare, aiming to keep them healthy and out of the hospital. This involves building long-term patient relationships through trust, care coordination, and a collaborative team environment focused on population health and primary care delivery.

Job Responsibility:

Provision of exceptional primary care
Conduct office visits for routine and acute issues
Administer Annual Wellness visits and health risk assessments
Care coordination with other providers and testing facilities
Population health leadership in coordination with Care Team
Assisting with phone triage and outreach
Educating patients on their health conditions and treatments
Conducting home visits as needed
Participating in promotional activities
Other duties as assigned

Requirements:

Master of Science in Nursing
Doctor of Nursing Practice
NP Certificate Program graduate
National certification in Family Nurse Practitioner or Adult-Gerontology Primary Care Nurse Practitioner or Adult Nurse Practitioner or Gerontological Nurse Practitioner
Active, non-probationary state Nurse Practitioner license
Active DEA license
US Work Authorization

Nice to have:

Experience in primary care such as internal medicine, geriatrics, or family medicine
Understanding of HCC documentation, ICD-10 Coding, and HRAs
Passion for teamwork and cross-functional collaboration
Interest in innovative models focused on population health
Bilingual proficiency
