Job Description:

CVS Health® is hiring a Part Time Clinician (Nurse Practitioner, Physician Assistant, or Physician) to provide In-Home Health Evaluations in the comfort of clients' homes. This role involves reviewing medication and medical history, conducting physical evaluations, performing diagnostic tests, and connecting clients to personalized healthcare.

Job Responsibility:

Provide In-Home Health Evaluations
Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification (NP/PAs)
Multi-state licenses (preferred)
Able to cover the following primary locations: Sacramento and surrounding area
