Job Description:

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. The job involves medication and medical history review, physical evaluations, and additional diagnostic tests as ordered by the health plan. Clinicians use iPads and intuitive clinical workflows, engaging with patients without prescribing medication or altering treatment regimens.

Job Responsibility:

Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
Travel locally and within licensed states, as needed

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification
Multi-state licenses (preferred)
Able to cover the following primary locations: Brainerd and surrounding area
