Job Description:

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Job Responsibility:

Provide In-Home Health Evaluations
Build trusted relationships to make people healthier and bridge gaps in care
Conduct virtual visits as needed
Provide Signify Health services including Diagnostic & Preventive Services

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification
Multi-state licenses (preferred)
Able to cover the following primary locations: Seattle and surrounding area
