Job Description:

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. The role involves engaging with people in their homes, answering health questions, conducting diagnostic tests such as diabetic eye exams, and improving health outcomes.

Job Responsibility:

Provide in-home health evaluations
Conduct virtual visits
Perform medication and medical history reviews
Conduct physical evaluations
Provide diagnostic and preventive services, such as diabetic eye exams and spirometry
Build trusted relationships with patients

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification
Multi-state licenses
