Job Description:

Signify Health is seeking a Part Time Clinician to provide In-Home Health Evaluations, connecting with people one-on-one to improve health care outcomes through medication and medical history reviews, physical evaluations, and diagnostic tests if ordered by the health plan.

Job Responsibility:

Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products
Bring your heart into every visit, joining a national network of purpose-driven clinicians
Be part of efforts to visit millions of people nationwide to provide connections to the right care

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification

Nice to have:

Multi-state licenses
