Nurse Practitioner or Physician Assistant – Primary Care at Home

This position works under the supervision of the Managing Physician for Primary Care at Home to help provide value based medical care and care coordination for homebound patients. The APC is involved in the planning, implementation, and evaluation of medical and nursing care for patients with complex care needs. The Care Manager – Nurse Practitioner assesses the physical and psycho-social condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories. The Care Manager orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs. The Care Manager may prescribe and/or recommend medical drug therapies or other forms of related therapeutic treatments and procedures in accordance with approved protocols/policy and with attention to efficacy, safety, and cost.
