The NP, Clinical Coordinator in the Heart Institute is a professional nurse who possesses a high degree of knowledge in cardiovascular nursing, and minimally invasive structural heart therapies including but not limited to: Transcatheter Aortic Valve Replacement (TAVR), Left Atrial Appendage (LAA) Closure, Percutaneous Mitral Valve Repair (PMVR), and Patent Foramen Ovale (PFO) closures. The NP will collaborate with the interventional cardiologists, electrophysiologists, cardiothoracic surgeons and cardiac anesthesiologists that are part of the Structural Heart Team. The NP is a leader in coordinating resources to facilitate the achievement of optimum health and wellness for each patient. Continuity is maintained across the continuum of care in all settings. The advanced knowledge and skills required for this role include clinical expertise in the specialty focus area, evidence-based practice, mentoring, consultation, collaboration, education, and transformational/change leadership; essential to providing excellent patient management.
Job Responsibility:
Actively participates in the development and sustainment of the Structural Heart program at Saddleback Medical Center to offer state of the art technology and personalized care to MemorialCare patients
Provides leadership and direction for the program, creates protocols, updates, and revises program components as needed
Demonstrates advanced knowledge and skill in assessment, diagnosis, management and evaluation of actual or potential needs of the patients with cardiac disease
Provides clinical support for the Structural Heart program in the inpatient & ambulatory care setting
Collaborates with the Structural Heart Team, to assure a smooth transition of patients during pre-admission, hospitalization, preparation for discharge and outpatient services to achieve the best possible care plan
Serves as a subject matter expert and resource for clinical staff caring for patients undergoing minimally invasive structural heart therapies
Demonstrates knowledge of current clinical research pertaining to structural heart therapies
Functions as a change agent in collaboration with the Structural Heart Team, to translate evidence/guidelines into practice in order to improve patient outcomes
Oversees and reports on outcomes published by CMS required data registry (if applicable)
Ensures compliance and best practice with the Structural Heart Program
Positive patient outcomes achieved with the utilization of developed programs and specialties
Educate and communicate current evidence-based practices to staff to ensure effective practices and effective problem solving
Effectively communicate with variety of professionals in the invasive and non-invasive cardiovascular settings
Presents professional presentations and group educations as to effectively educate professional staff
Ability to be at work and be on time

Requirements:
Minimum 3 years' experience as an RN/NP in an adult acute care cardiology setting
Clinical Coordinator experience preferred
Experience in quality improvement theory, change theory, adult learning principles, and team building
Previous experience in the development of evidence based clinical protocols
Knowledgeable of current research and strength in translating research into everyday practice
Previous experience in program development
Knowledge of quality measures and previous participation in quality improvement projects
Graduate of an accredited Nurse Practitioner Program
Valid California License as a Nurse Practitioner
Valid Furnishing License
Valid DEA License-preferred
Board certification in area of specialty by the American Academy of Nurse Practitioners or American Nursing Credentialing Center
A current certification from the American Heart Association in Basic Life Support (BLS)
A current certification from the American Heart Association in Advanced Cardiac Life Support (ACLS)
Malpractice Insurance Coverage as required by the Medical Staff Bylaws
