The Advanced Practice Nurse - Nurse Practitioner (NP) will apply specialized knowledge, skills and responsibilities in providing and advancing quality care for the cardiovascular patient. The individual will see patients in clinic as well as remote follow up pre and post procedure. They will help arrange diagnostic testing pre and post procedure. The Advanced Practice Nurse uses an advanced cardiovascular knowledge base, physical assessment skills, diagnostic interventions and a holistic perspective to participate in the plan of care for the patient, in collaboration with the patient, the family, physicians and the interdisciplinary team. This team provides care to a complex patient population. The Advanced Practice Nurse will be responsible for managing comorbidities within this population to advance the plan of care. They will support the patient and their journey across the continuum of care which could include both inpatient and outpatient settings. The Advanced Practice Nurse directs the development and implementation of basic and advanced educational programs for patients, families, staff, other health professionals and self, guided by concepts from relevant educational theory. The Advanced Practice Nurse will play a significant role in the development of evidence based practice for the cardiovascular patient population. They will also collaborate with the research coordinator to support clinical trials within the program. The Advanced Practice Nurse will report to both the Clinical Director – C&V Program and the Chief of Nursing Practice.

Requirements:
Current Certificate of Registration with the College of Nurses of Ontario
Membership in provincial, federal or international professional associations
Adult Nurse Practitioner Masters Degree OR Master Degree in Nursing and NP – Adult Diploma
Recent (minimum 5 years) clinical experience in cardiovascular nursing
Demonstrated knowledge and understanding of community services and resources
Demonstrated excellent interpersonal and communication skills
Demonstrated ability to promote professional development
Demonstrated leadership and critical thinking skills
Demonstrated commitment to patient and family centered care
Demonstrated ability for independent action, initiative, problem solving, organization and prioritization
