Requirements

Registered Nurse license issued by the state in which the team member practices
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
A valid drivers license issued by the Division of Motor Vehicles
Associate's Degree in Nursing
Typically requires 2 years of experience in nursing
Proficiency in clinical skills with the ability to work under direction and make sound judgments
Excellent communication (written and verbal) and interpersonal skills
Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians
Demonstrated ability to effectively make critical, independent decisions
Good organization, prioritization and problem solving skills

Job Responsibility

Performs all scheduled visits and collaborates with members of the interdisciplinary team as necessary
Conducts thorough physical, psychosocial and environmental assessments of assigned patients and updates plan of care where appropriate and as a result of visit finding
Follows a care plan, which identifies problems (nursing diagnosis) and evaluates outcomes within a certain time frame
Communicates with physicians to confirm and update patient's plan of treatment that may be necessary as a result of assigned visits
Communicates those changes to the primary registered nurse
Provides on-going assessment of patient response to treatments, medications and teaching and applies interventions as appropriate
Updates and revises care plan as indicated
Schedules patients and case conferences with primary registered nurse
Assists with the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge
Accurately documents and updates patient records according to agency, state and federal guidelines

Current License as Nurse Practitioner in the State of Alabama
Minimum of one (1) year medical/surgical experience
Current, valid Alabama Driver's License

Valid CPR certification

Job Responsibility

Performs the face to face encounter within 30 calendar days prior to start of the 3rd benefit period and all subsequent re-cert periods
Gathers and documents clinical findings to determine continued hospice eligibility
Shares the clinical findings with the hospice medical director
Attests in writing that he/she performed the face to face encounter and that these findings were provided to the certifying physician
Communicates with Clinical Director and RN Case Manager prior to visits as to pertinent details of patient's disease process and outcome of the actual face to face visit
May make patient consults and visits as appropriate and as requested
May attend interdisciplinary team meetings as appropriate
Provides in service education as requested
Maintains sound privacy and security practices and prevents privacy or security breaches
If breach occurs, takes corrective action
