Serves as the primary contact for patients, families, and healthcare providers, facilitating communication and ensuring seamless coordination of care in both inpatient and outpatient settings.
Responsibilities include triaging patient inquiries, educating patients and families about treatment options and changes, and collaborating with the HBC team to optimize patient outcomes.
