We are recruiting for an experienced Nurse Case Manager with tracheostomy expertise to join a specialist provider of complex care services in South London. This is a full-time, Monday–Friday position (9am–5pm), based from a South West London head office with regular community visits. This role offers the opportunity to work closely with adults and children with highly complex clinical needs, supporting them to live safely and independently in their own homes through bespoke, nurse-led care packages.
Job Responsibility:
Manage a personal caseload of complex community care packages
Provide regular face-to-face clinical support to clients and their families
Create, implement, and regularly review personalised care plans
Train, mentor, and clinically support a core team of carers
Deliver competency-based clinical training including tracheostomy and enteral feeding
Work collaboratively with multidisciplinary teams and external health professionals
Monitor care quality, risk, and compliance across all assigned packages
Respond flexibly to changing priorities while meeting professional deadlines
Split working time between community settings and head office base
Requirements:
Registered Nurse with an active NMC PIN
Minimum 12 months’ post-registration experience in the UK
Tracheostomy experience highly desirable
Ventilation and ITU/HDU experience advantageous
Enteral feeding experience essential
Knowledge of spinal injuries strongly desirable
Strong written and verbal communication skills
Confident in autonomous caseload management
Supportive leadership style with a strong coaching approach
Ability to build trust with clients, families, and care teams
Flexible, proactive, and solutions-focused working style
Must have the right to work in the UK (no sponsorship available)
