As a Hospice at Home Nursing Associate, you will be part of the Rainbows team delivering clinical care to babies, children and young people within their own homes, offering essential support to parents and carers. The role includes assessment within your scope of practice and delivering planned day‑to‑day care based on individual needs. Hospice at Home operates seven days a week.
Job Responsibility:
Working in children and young people’s homes as part of the Hospice at Home service
Supporting the Registered Nurse in delivering safe, person‑centred care, often working independently with an agreed plan of care
Delivering excellent short break, palliative and end‑of‑life care, including care after death
Participating in assessment, planning, implementation and evaluation of care under the direction of the Registered Nurse
Engaging with families to understand needs, preferences and choices, contributing to risk assessments and care planning
Providing a high standard of holistic care
Adhering to the NMC Code and working within your scope of practice
Taking responsibility for medication administration, storage and return in line with policy
Maintaining accurate, contemporaneous records and handling information in line with legislation and policy
Communicating sensitive information effectively and using appropriate strategies to support person‑centred care, equality and diversity
Using SystmOne appropriately
Acting independently and collaboratively to uphold the rights of individuals
Liaising with the wider multidisciplinary team and external agencies to ensure continuity of care
Maintaining confidentiality at all times.
Requirements:
Registered Nursing Associate with current NMC registration
Nursing Associate foundation degree
Experience with children and young people (0–19) with complex needs
Minimum one year post-registration experience
Willingness to undertake further study
Experience working within a team and with babies/children/young adults in healthcare, community or social settings
Understanding of NMC standards, evidence-based practice and holistic care
Effective verbal and non-verbal communication skills
Flexibility and adaptability to unplanned situations
Ability to seek advice and escalate concerns appropriately
Understanding of procedures, treatment plans, equal opportunities and safeguarding
Basic IT skills
Valid UK driving licence.
