Np / Pa Primary Care - Senior Health Clinic

The Senior Health Clinic, located at 1823 Camino de Salud, is looking for an Advanced Practice Provider (APP) experienced in working with Seniors to provide comprehensive, patient-centered primary care services in a collaborative clinical environment. Function in a primary care setting in the role of practitioner, mentor, consultant, educator, researcher and manager. Establish goals and priorities in accordance with patient and staff needs and organizational objectives. Ensure adherence to Hospitals and departmental policies and procedures. Patient care - Women's Specialties: Ages of patients served restricted to adolescent, adult and geriatric age groups. Family Practice Clinic: Ages of patients served may include neonate, pediatric, adolescent, adult and geriatric age groups. Senior Health Clinic: Ages of patients served restricted to geriatric age groups.
