Job Description:

Start your NP career in the future of healthcare! Oak Street Health’s NP Fellowship in Adult Primary Care collaborates with the University of Michigan School of Nursing. This 12-month paid fellowship aims to equip fellows with skills and confidence for serving older adults within a value-based care model. Fellows spend time building patient panels, learning from preceptors, and enjoying structured didactic learning. Fellows commit to a year of employment after completing the program and may continue working with Oak Street Health.

Job Responsibility:

Building own patient panel
Learning from a multitude of preceptors
Engaging in didactic content that builds on graduate training
Collaborating with interprofessional care teams
Driving clinical excellence.

Requirements:

Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between December 2023 and May 2025
Currently residing in or willing to relocate to program location for at least the next two years
Have not yet worked as an NP
Minimum of one year of experience, within the last five years, working as an adult RN
Minimum GPA of 3.0 in NP program
Currently holds or will hold by 9/15/2025 an active, non-probationary state NP license
A genuine passion for adult primary care
A supportive attitude toward our patient population of older adults
Embrace teamwork and the opportunity to collaborate with brilliant colleagues

Spanish language skills are strongly preferred, depending on clinic location.
