Job Description:

The House Calls Mobile Operations Coordinator serves as an integral part of the in-home care team, coordinating logistics and scheduling for patients and teams, ensuring wrap-around clinical care for patients, and providing telephonic support. This role is vital to the innovative home-based care program.

Job Responsibility:

Prepare daily visit schedules
Answer incoming calls
Complete patient appointment confirmation calls
Conduct outbound calls
Monitor incoming faxes
Manage patient panel registry
Coordinate patient transition back to center
Request patient medical records
Monitor patient admissions and schedule post-discharge visits
Follow-up on DME and Home Health orders
Follow-up on Specialty Appointments
Participate in daily huddles and weekly interdisciplinary team meetings

Requirements:

Strong computer skills
Professional phone etiquette
Bilingual a plus
Medical background preferred
Experience in customer service setting strongly preferred
Some college preferred
US work authorization
